To my knowledge this might be due to translation difficulties (in German for example “Efficacy” is usually translated as “Wirksamkeit” and “Effectiveness” as “Wirksamkeit unter Alltagsbedingungen”, which is then too often shortened leaving “Wirksamkeit” alone again).
